Water's Edge Care Center has the capability of
providing care to individuals with a range of
physiological conditions requiring orthopedic
intervention and/or management. Individual candidates
must have the ability to actively participate
in rehabilitation 5-7 days a week. Sufficient
support systems with a pre-determined discharge
placement is essential to facilitate a successful
and functional outcome. This subacute program
is designed for those individuals who require
more intensive therapy than is practical at home
or in an ambulatory setting.

Program Description and Goals:
- Restoration and/or maximization of functional abilities
- Mobilization education and training for all levels
- Physical strengthening and endurance
- Monitoring of surgical incision, CMS, and joint integrity
- Pain management
- Activities of daily living, education and training
- Assistive and adaptive equipment provided to facilitate independence
- Pre-discharge home assessment with recommendations to improve
quality of life
- Discharge planning and coordination of services to home
- Orthopedic consultation and follow-up appointment as indicated

This program is designed to provide medical management and rehabilitation
to individuals with a new onset of neurological condition(s) effecting
multilevels of functioning. An interdisciplinary team approach provides
comprehensive treatment to facilitate maximum independence and compensatory
training for both the patient and care givers. Individuals being
referred for this program should be medically stable and exhibit
potential for functional gains. Patients should be able to actively
participate in skilled therapies 5 -7 days a week.

- Cerebrovascular accident
- Neuropathies
- TIA
- Multiple Sclerosis
- Parkinson's Disease
- Degenerative neurological conditions
- Guillain-Barre syndrome
- Brain tumor/lesions
Program Description and Goals:
- Establish goals for swallowing and feeding abilities as indicated
- Establish nutritional requirements and enteral tube feedings
as indicated
- Establish bowel and bladder retraining goals and needs
- Determine compensatory strategies for perceptual, visual, motor
planning and ADL deficits by OTR
- Determine compensatory strategies for cognitive and linguistic
deficits by S/LP
- Provide physical therapy for physical deconditioning, paralysis,
and mobility skill needs
- Physiatry consultation for physical medicine and rehabilitation
recommendations
- Determine support service needs and recommendations for discharge
planning
- Home assessment to determine adaptive equipment and home care
services
- Discharge planning and coordination of follow-up services

post surgical interventions. The program is designed
for individuals with complex care issues requiring
aggressive skilled nursing care and an interdisciplinary
approach to care. Individuals should be generally
stable and have the ability to actively participate
with the integration of medical services and rehabilitation.
Diagnosis and Related Conditions considered for the program:
- Colostomy
- Infected wounds/joints
- Ileostomy
- Tracheostomy
- Amputations
- Major surgical procedures (status post)
- Gastrostomy feeding tubes
- Status post flap/graft procedures
- Jejunostomy feeding tubes
- Multiple trauma
Program Description and Goals:
- Monitoring and management of surgical wounds
- IV therapy as prescribed by M.D.
- Nutritional consultation and education on enteral feedings,
TPN, wound care etc.
- Energy conservation techniques and mobility training as indicated
- Education and training of ostomy care
- Education and training of enteral tube feeding care and management
- Pain management
- Respiratory treatments and stabilization of post surgical conditions
- Develop compensatory strategies for activities of daily living
and all levels of mobility
- Patient training and family education in preparation for home
- Discharge planning and coordination of follow up services

This program is designed to provide medical management
and rehabilitation to individuals with compromised respiratory conditions.
Individuals may present with both a new onset or an exacerbation
of a chronic respiratory or cardiac related condition and/or a past
surgical condition. Candidates being considered for this program
should have medical stability and have the capability to actively
participate in a multi-modality therapy program.
Diagnosis and Related conditions considered for the
program:
- Chronic Obstructive Pulmonary Disease
- Lung complications post surgical procedures
- Pre and postoperative lung reduction surgery
- Bronchiectasis
- Asthma
- Cystic Fibrosis
- Neuromuscular disease
- Sleep apnea
Program Description and Goals:
- Establish activity orders and precautions per M.D. direction
- Assessment of lung sounds and pulse oximetry with oxygen titration
- Develop compensatory strategies and education for energy conservation
- Establish nutritional needs and supplements as determined by
dietician
- Provide physical therapy treatment for mobility, strength and
endurance
- Provide occupational therapy treatment for activities of daily
living
- Provide educational materials for daily structure and guidelines
to patients and family
- Pulmonary consultation and follow up appointments as scheduled
- Dyspnea management and panic control
- Improve quality of life
- Promote care throughout continuum and aim to reduce hospitalizations
- Discharge planning and coordination of follow up services
