Water’s Edge Center for Nursing & Rehabilitation recognizes the importance of intensive short-term rehabilitation. We have developed our “Passport Rehabilitation” program specifically designed to meet the needs of individuals requiring short-term rehabilitative stay following a hospitalization or an acute medical episode. While on your short-term “journey” with us, please take full advantage of the amenities and services our facility offers. Our short-term rehabilitation unit consists of a dedicated floor and a spacious rehabilitation gymnasium for those residents admitted for short-term intensive rehabilitation. Our high number of residents discharged home following our short-term rehabilitation program is indicative of the commitment and dedication of our specially trained staff.

Our rehabilitation department, consisting of physical, occupational and speech therapy, strives to have all patients functioning to the best of their ability in the shortest period of time. The following rehabilitative services are available as needed:

Candidates for short-term therapy include, but are not limited to, residents diagnosed with:

• Cerebrovascular Accidents (Strokes)
• Multiple Sclerosis
• Parkinson’s Disease
• Spinal Cord Injuries
• Fractures
• Amputations
• Chronic Obstructive Pulmonary Disease
• Congestive Heart Failure
• General deconditioning secondary to medical and surgical conditions

  Some of the amenities offered in the Passport Program found at our facilities, in addition to a Clinical Nurse Specialist to oversee your entire stay, the following amenities are offered to make your stay more comfortable:

  • Telephone

  • DVD player / library

  • Wireless Internet (Wi-Fi) access

  • Daily newspaper delivery

  • Magazines

  • Selective Dining and/or Room Service

   

This program is designed to meet the special needs of Alzheimer’s/Dementia residents in a safe, caring and secure environment. Our newly renovated and spacious open floor design fosters a positive and warm atmosphere, encouraging residents from all walks of life tom participate in recreational activities. Our goal is to help residents with Alzheimer’s or related dementias maintain their current level of functioning.

Water’s Edge Center for Nursing & Rehabilitation has specialized in delivering quality long term care to residents for more than 25 years. Upon admission, each resident is evaluated by our interdisciplinary team. The team develops a comprehensive care plan which includes all medical, nursing, rehabilitation, nutritional, psychosocial and therapeutic needs. The resident and family members are invited to participate in the comprehensive care planning process. Each care plan is individualized, taking into account a resident’s likes and dislikes, hobbies, interests and capacities.

Each resident is evaluated upon admission, based on need, for rehabilitative services in physical, occupational and/or speech therapy. Thereafter, intensive, restorative or maintenance therapies are carried out by a team of therapists in conjunction with the resident’s plan of care. Our goal is to assist each resident with the means to maintain his/her optimum level of functioning.
 

Water’s Edge Center for Nursing & Rehabilitation recognizes the need for caregivers to have time for themselves, to relieve stress and prevent burnout. Our scheduled short-term respite care program provides relief to the caregiver and quality care to the patient.

Our scheduled short-term respite care will be offered as an addition to our current long-term skilled nursing care and short-term rehabilitation programs.

We offer 24-hour care to applicants seeking a limited stay in our facility.
